[Fmpro] ASCAP promo checklist?

Hey Guys, 
Maybe someone could shed some light on something...
I called ASCAP now a couple of times, I explained that I scored
a film and that there were a number of promos that ran on IFC
for the film, some were just trailers and other were long form
10 or 15 minute interviews and clips...like making of type of stuff.

 I was told I need the ³promo checklist,² each time I called I was
told that I¹d get the list emailed to me and neither time did it
come.

Does anyone have this checklist?

And do promos and trailers need to have cue sheets sent in...
When does something change from a commercial to a program...
For example HBO ³behind the scenes of ...²
