Yep. I think I remember using one of my piece's live performances with a dance troupe for this, and
sending in one of the programs from it I'd saved as proof. This was over ten years ago, so I'm not
even sure if that's required now.

Tax ID would be your SS#, since you haven't registered it as a separate tax entity. Stakeholder
would be you, and the where and when would be where you live and when you decided to set it up. I
seem to remember that being how it worked out when I established my own publishing, though I did
have my own licensed business which was a sole proprietership at the time. It seems like it should
be more complicated, but it really isn't. :)
